Position Overview
We're seeking a skilled Population Health Administrator to join our dynamic team. This role is critical in developing, implementing, and managing our organization-wide strategy for maximizing quality and financial achievement in Population Health and Value-Based Care initiatives.
Key Responsibilities
 Lead high-impact, organization-wide population health initiatives
 Convene and lead cross-functional teams on broad-scope projects using advanced performance improvement models
 Develop and execute comprehensive project plans, managing multiple projects simultaneously
 Create annual plans and proposals to meet program goals and targets
 Identify and address potential threats to value-based program performance
 Collaborate with clinical and operational leaders to develop systems for monitoring key performance indicators
 Analyze complex data, interpret findings, and develop actionable reports and presentations
 Serve as a subject matter expert in population health, providing guidance on strategies and policies
Qualifications
 Current CA RN license
 Bachelor's degree required; Master's degree in a related field preferred
 5+ years of related experience in healthcare, particularly in quality improvement and population health
 Strong knowledge of MS Office Suite, especially PowerPoint, Excel, and Visio
 In-depth understanding of Performance Improvement standards, Advanced Payment Models, and Value-Based Care models
 Excellent analytical, critical thinking, and problem-solving skills
 Outstanding verbal and written communication skills
 Proven ability to lead effective teams and manage multiple projects
 Detail-oriented with the ability to see the big picture
 Certification(s) in a related field
Preferred Skills
 Experience with EPIC electronic health record system
 Demonstrated data analytics experience
 Healthcare setting experience with focused quality improvement projects
