SeniorsPlus is seeking full-time Home Visitors to assist clients in making informed choices to live safely and independently in their communities. One position is available in each of the following Regions: Northern, Central, and Southern Maine.
What YouÂll Do as a Home Visitor:
 Perform home visits to clients and ensure timely, accurate documentation is communicated to Care Coordinators.
 Assess clients health, welfare, and unmet needs while developing backup plans and goals to support their independence.
 Collaborate with Care Coordinators to ensure effective, efficient, and coordinated services.
 Evaluate the clientÂs plan of care to ensure continuity and effectiveness.
 Positively impact the well-being of disabled children, adults, and older adults participating in at-home, long-term care programs.
 Travel throughout assigned regions as needed (mileage is reimbursable).

Qualifications: Â Education: BachelorÂs degree in Social Services, Human Services, or Health-related field (Required). Â Experience: At least 1 year of customer service experience (Required). Â Strong computer and organizational skills (Required). Â Familiarity with the long-term care system is a plus. Â Must have a valid driverÂs license and reliable transportation.
Schedule:
 Full-time, 8-hour shifts
 Day shift with occasional weekend availability
 Travel is required (mileage reimbursement is provided).
