Home Visitors – Full Time / Remote Positions

SeniorsPlus is seeking full-time Home Visitors to assist clients in making informed choices to live safely and independently in their communities. One position is available in each of the following Regions: Northern, Central, and Southern Maine.

What You’ll Do as a Home Visitor:
• Perform home visits to clients and ensure timely, accurate documentation is communicated to Care Coordinators.
• Assess clients’ health, welfare, and unmet needs while developing backup plans and goals to support their independence.
• Collaborate with Care Coordinators to ensure effective, efficient, and coordinated services.
• Evaluate the client’s plan of care to ensure continuity and effectiveness.
• Positively impact the well-being of disabled children, adults, and older adults participating in at-home, long-term care programs.
• Travel throughout assigned regions as needed (mileage is reimbursable).

Qualifications: • Education: Bachelor’s degree in Social Services, Human Services, or Health-related field (Required). • Experience: At least 1 year of customer service experience (Required). • Strong computer and organizational skills (Required). • Familiarity with the long-term care system is a plus. • Must have a valid driver’s license and reliable transportation.

Schedule:
• Full-time, 8-hour shifts
• Day shift with occasional weekend availability
• Travel is required (mileage reimbursement is provided).
