ESSENTIAL FUNCTIONS:
 Engage with individuals within their residence providing support to lead as independent lives as possible

 Provide direct supervision of staff, serves as a mentor, and guide best practices for individuals serves.

 Assure individualized program plans and data are maintained
 Provides supports to individuals by assisting in the development and implementation of all support plans, including spiritual life supports, assisting individuals in achieving their;
 Assist individuals in maintaining safe and healthy lifestyles, monitor for changes in their general health, appearance, behavior, and patterns of activities.
 Develop a relationship with individuals served that promotes their best interests, assists in achieving their goals and supports an independence in their lives.
 Reports and documents routine and unusual events including completing daily activity sheets, accident reports, case notes etc. in a concise, understandable, and legible manner, and meets agency, federal, and state regulatory requirements.
 Responsible for monitoring and maintaining a clean, safe, and secure environment for individuals at all times, including but not limited to programs/residences and vehicles.
 Plan and support individuals in daily activities, which may include, but are not limited to, household chores, personal care, socialization, recreational activities, learning activities, physical fitness activities and money management.
 Oversees individualÂs fiscal management in accordance to Agency/Residential policies.
 Provide training and assistance to individuals in making healthy dietary choices, and following any pre-established order by medical professionals.
 Maintains open communication and cooperative work effort with direct care professionals to ensure quality and continuity of support to individuals served.
 Assure staff are meeting training requirements and certifications as required.
 Ensures that individuals exercise their rights and that rights are not restricted without due process.
 Works cooperatively within the team setting with local and state service providers ensuring self-determination of the individuals served.
 Works under the manager to oversee program/residential operations and to assure compliance with all agency, local, state and federal regulations (including licensing and other inspections conducted by funding sources).
 Establishes and works cooperatively with all agency and community contacts
 Provides mentoring for staff in all areas of service delivery, including but not limited to interactions, communication, networking and supports.
 Rotating on call responsibilities with manager.
 Managerial oversight as part of the staffing compliment to ensure the quality of the services being provided. Key areas of focus include the implementation of programs/teaching strategies, data collection adherence, and supervision guideline adherence
 Maintains effective communication and reporting to both the manager and director on a regular basis.
 Ensure all staff are abiding by the protocols and polices set forth by DDS, the agency and the Individuals Plans/Guidelines.
 Oversee staff shift duties as it relates to the residents, emergency situation, facility maintenance, sanitation, documentation and resident accountability.
 Assist in ensuring new staff complete new hire training
 Performs other duties as assigned/designated by the manager/director level
PHYSICAL REQUIREMENTS
 Lifting a minimum of 50 pounds
 Standing, bending, twisting  neck, waist, lifting, pushing and pulling (e.g. wheelchairs), driving, walking
 Physical Management Techniques (PMT) to support behavioral needs
 Administer CPR/1st Aid whenever necessary
 Daily computer and written documentation required
 Transferring individuals from: (with or without medical lifts) to bed to floor/wheelchair, floor/wheelchair to bed, in and out of shower/bath, into and out of vehicles
 Assisting individuals with bathing/showering, changing individualÂs briefs,
 Housecleaning duties, e.g. laundry, vacuuming, cleaning showers, tubs, dishes, taking out garbage
 Preparing meals
 Conducting required evacuation drills, earthquake training, and other safety training
 Assisting with hand washing, changing soiled clothing, brushing teeth, assisting with bathroom responsibilities
REQUIREMENTS:
AssociateÂs degree in related field and 2 years of experience in mental health, behavioral strategies and interventions, developmental disability, and autism required. Two years of prior supervisory experience required. 5 years of experience in mental health or human service field may be substituted for education requirement. Proof of valid driverÂs license and driving record in good standing. Must have or obtain CPR, first aid, Med Cert, PMT and all other training requirements

Pay: $18.50 - $20.25 per hour
