Do you have what it takes to be a Care Coordinator:
 Completion of a Medical Assisting or LPN education program required
 CMA/RMA or LPN required
 Experience with quality reporting preferred
A day in the life of a HSO Care Coordinator:
 Provide telephone outreach and in-person consultation to members identified as high risk
 Assist in identifying gaps in care
 Collaborate with healthcare team to optimize patient adherence to medical plan of care
 Proactively outreach to members newly diagnosed with a targeted chronic condition
 Educate patient and family members about preventative screenings and identify barriers to care
 Interact with healthcare team
