About the position

The Care Team Associate position at Optum involves providing administrative support and customer service within a community-based medical group. The role is essential for ensuring effective communication and coordination of care for members, while adhering to regulatory requirements. This full-time position offers a hybrid work environment, combining both onsite and remote work, and includes comprehensive training to equip associates with the necessary skills for success.

Responsibilities
• Document all member interactions in compliance with documentation requirements for Model of Care, NCQA, and CMS regulations.

• Receive and respond to incoming Care Coordination inquiries from various communication channels.
,
• Coordinate and assist in monitoring documentation Care Management queues for various referrals.
,
• Process Care Management requests and prepare administrative support for clinical staff.
,
• Conduct inbound and outbound calls for program requirements, including member scheduling and surveys.
,
• Prepare daily INPT and Post-Acute Census for CM referrals and inform the Care Management Manager of outstanding cases.
,
• Complete timely data entry of member contact information into software applications.
,
• Provide clerical and administrative support to clinical staff and managers for special projects and reporting needs.
,
• Deliver excellent customer service to all internal and external customers.
,
• Attend required meetings and participate in ad hoc committees as needed.
,
• Maintain knowledge of health plan benefits, network, NCQA regulations, and health plan policies.
,
• Identify opportunities for process improvement in member care.
,
• Maintain strict confidentiality and adhere to all department policies.

Requirements
• High School Diploma / GED or equivalent work experience.
,
• Must be 18 years or older.
,
• Medical assistant certification.
,
• 2+ years of administrative support experience.
,
• Knowledge of medical terminology.
,
• Advanced knowledge of Microsoft Office products, including Excel, Word, and Outlook.
,
• Ability to toggle between multiple IT systems and platforms.
,
• Ability to work normal business hours of 8:00am - 4:30pm EST, with occasional overtime.

Nice-to-haves
• 2+ years of experience in a physician's clinic or hospital.
,
• 2+ years of experience working in a medical care setting as a receptionist or medical assistant.
,
• Ability to prioritize, plan, and handle multiple tasks simultaneously.
,
• Ability to work independently with some supervision.
,
• Ability to maintain professionalism in conduct and appearance.
