Standard Shift- M- TH 8:30am/5:00pm F- 8:30am -5:00pm- Rotating Saturdays and Sundays

Primary Responsibilities:  Exception to meet KPI 50 calls a day  Assess patient's health status and recommend care based on clinical judgment and protocols  Identify potential care and/or provider gaps  Coach patients on treatment alternatives  Coordinate services and referrals to health programs and community services  Assesses and triage immediate health concerns  Initiate the plan of treatment/care pathway based upon the analysis of information obtained and within scope of practice and certification  Accurately communicate (written/verbal) patient status information to prescribing physician and case manager in timely manner and provide necessary clinical information to Optum pharmacists and other healthcare professionals as applicable  Provide care and services to assigned caseload in an efficient, organized manner. Maintain flexibility in assignments and scheduling

Required Qualifications:  Current, unrestricted RN licensure (include state specific license)  2 years of clinical experience  Proven track record of excellent Clinical record documentation  Technology knowledge to include desktop/laptop computer proficiency; use of analytical software, Microsoft Office, Outlook (internet and email). Attend in-services on applicable
Preferred Qualifications:  BachelorÂs degree in nursing (BSN)  1 years Triage phone experience/remote  Basic knowledge of theories, practices and procedures in a function or skill  Knowledge of how to navigate Clear Triage  Proven to perform routine or structured work  Proven to respond to routine or standard requests  Proven to use existing procedures and facts to solve routine problems or conduct routine analyses  Proven to depend on others for instruction, guidance, or direction  Proven exceptional communication skills (written and verbal)  All employees working remotely will be required to adhere to UnitedHealth GroupÂs Telecommuter Policy
The hourly range for this role is $28.61 to $56.06 per hour based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
