The Care Coordinator/Manager position at Evan May, MD, PLLC is a part-time, flexible role focused on managing chronic care for patients in a small Internal Medicine & Cardiology practice. The role involves remote monitoring programs, patient communication, and coordination of care, with a significant portion of the work being done from home. Responsibilities
 Provide communication support for patients with chronic conditions between appointments.

 Organize and manage remote patient monitoring (RPM) and chronic care management (CCM) programs.

 Contact patients and families to introduce and reinforce the purpose of RPM and CCM programs.
,
 Enroll and coordinate the purchase of appropriate devices for patients as ordered by Dr. May.
,
 Log onto EMR and other software platforms to view data and coordinate care.
,
 Call patients regarding their home data collection and reaffirm teaching from Dr. May.
,
 Communicate concerning data trends with Dr. May for treatment plan adjustments.
,
 Establish long-term trusting relationships with patients.
,
 Ensure revenue billing cycles for CCM/RPM programs are met.
,
 Schedule encounters in EMR and coordinate billing cycles with the in-house biller.
,
 Be available for blood draws/injections during office hours as needed.
,
 Call in medication refills when necessary.
,
 Troubleshoot and triage patient messages, escalating to Dr. May when needed.
,
 Work closely with office staff and Dr. May to provide high-quality care.
,
 Cross-train with MA and office manager to ensure operational continuity.
,
 Maintain accuracy of patient demographic and insurance information.
,
 Ensure documentation is accurate and reflective of care delivered.

 Promote patient communication platform and assist with app setup.
,
 Schedule patients for ancillary testing or laboratory studies.
,
 Coordinate referrals to specialists and other healthcare providers.
,
 Utilize computer programs for patient inquiries and scheduling as needed.
,
 Communicate with pharmaceutical and device representatives and contract sonographers.
,
 Follow all OSHA, HIPAA, and company policies and procedures.
Requirements
 LVN, LPN, or RN certification.
,
 At least two years of experience in a patient care setting.

 Excellent customer service skills.
