The Licensed Vocational Nurse (LVN) will assist Neighborhood Healthcare clinicians to support patients seeking womenÂs health services. This role will verify and document quality of care related to womenÂs health services and manage cancer screenings, follow up, and treatment.
Roles & Responsibilities
Health Navigator:
 Manages a case load of patients for preventative womenÂs health screenings, including following-up and tracking of abnormal screening results
 Conducts outreach to patients due for screenings via phone, mail, or web-portal
 Schedules patients for follow-up appointments in accordance with preventative and maintenance care schedules
 Utilizes internal pap/breast tool to track abnormal and unsatisfactory results
 Conducts chart reviews to ensure appropriate American Cancer Society (ACS) guidelines are consistent with recommended patient management plans, including communicating the management plan and results to patients
 Monitors referrals tracking of abnormal pathology and referrals, such as colposcopies and endometrial biopsies
 Tracks and follows up on womenÂs health specialties referrals, such as breast surgeon, oncology, and gynecology
 Serves as case manager to support patients, streamline patient inquiries, and educate patients on recommendations to ensure appropriate and timely follow ups
 Complies with Family PACT and Every Woman Counts (EWC) program guidelines for patient notifications and tracking to complete and document entire patient cycles
 Supports mammogram mobile events, including scheduling, authorizations, result tracking, and documentation
 Notifies patients of needed follow ups, referrals, treatments, and other services
 Orders screenings, diagnostics imaging, labs, and tests for patients by following clinical guidelines
Recordkeeping:
 Completes population health management tasks, such as appointment scheduling, recalls, actions, web-portal call backs, and addressing telephone encounters.
 Creates and maintains trackers of patients that have been contacted and completed office visits
 Completes eClinicalWorks notes for all care coordination and services provided
 Prioritizes activities according to need and required follow-ups, such as outreach calls, chart reviews, ROR requests for health plan patients
 Creates reports to track progress of assigned tasks by keeping a log of discussion topics for care team and supervisor meetings
Requirements
Education/Experience: High school diploma/GED required. Valid CA Licensed Vocational Nurse (LVN) license required. One-year LVN experience required; experience in ambulatory clinic and/or community health setting preferred.
Additional Qualifications (Knowledge, Skills, and Abilities)
 Bilingual (English/Arabic) or (English/Spanish) preferred.
 Excellent verbal and written communication skills, including superior composition, typing and proofreading skills
 Ability to interpret a variety of instructions in written, oral, diagram, or schedule form
 Knowledgeable about and experience with basic nursing procedures, nursing skills
 Ability to work in a faced paced busy clinic environment
 Ability to successfully manage multiple tasks simultaneously
 Excellent planning and organizational ability
 Ability to work as part of a team as well as independently
 Ability to work with highly confidential information in a professional and ethical manner
Neighborhood Healthcare requires employees to be fully vaccinated (including a booster dose) against COVID-1 and demonstrate proof of vaccination/booster upon hire or receive approval for an exemption based on a medical reason, disability, or religious belief. Proof of booster dose will be contingent upon booster eligibility.
Pay range: $30.00 - $36.00 per hour, depending on experience. This role is also eligible for a $2,500 signing bonus.
