About the position

The Care Manager - Telephonic Nurse position at CenterWell involves providing support to patients transitioning into and out of home health services. This role is crucial in ensuring optimal continuity of care, requiring a deep understanding of post-acute care options and effective communication with clinical teams. The position is part-time, primarily working weekends and one weeknight, and is conducted via telephonic interactions with patients.

Responsibilities
• Act as CenterWell Home Health representative for patients discharged from service or needing post-acute services.

• Navigate healthcare options, including care services, Medicare coverage, and billing issues.
,
• Make clinical level of care determinations based on discussions and medical records.
,
• Act as a customer advocate throughout the referral process to ensure timely responses and maximize referral to admission conversion rates.
,
• Maintain awareness of department performance objectives and assure patient satisfaction goals are met.
,
• Assist in the admissions process for patients meeting requirements.
,
• Provide education regarding Home Health, Hospice, and Palliative Care Services.
,
• Participate in special projects and perform other duties as assigned.

Requirements
• Licensed Registered Nurse (RN) with compact state licensure and no disciplinary action.
,
• 3 - 5 years of clinical acute care experience.
,
• Comprehensive knowledge of Microsoft Office applications including Word, Excel, Outlook, Teams, and One Note.
,
• Passion for contributing to an organization focused on improving consumer experiences.
,
• High-speed internet connection (no hotspot, DSL, or satellite).

Nice-to-haves
• Experience with case management, discharge planning, and patient education for adult acute care.

• Managed care experience.

• Home Health Care experience.
