LVN HSS Coordinator - STAR or CHIP - Hybrid in Texas

About the position

The LVN HSS Coordinator role at UnitedHealthcare involves working with STAR and CHIP members who have Special Healthcare Needs. Responsibilities
• Assess, plan, and implement individualized care strategies for patients.

• Identify and initiate referrals for social service programs, including financial and community services.

• Manage the care plan throughout the continuum of care as a single point of contact.

• Communicate health-related information to stakeholders to ensure quality coordinated care.

• Advocate for patients and families to ensure their needs and choices are represented.

Requirements
• High School Diploma/GED (or higher).

• Current, unrestricted LPN/LVN license in the state of Texas.

• 3+ years of clinical experience in a health care role.

• Intermediate level of Microsoft Office proficiency, including Word, Excel, and Outlook.
,
• Ability to travel up to 10% of the time for team meetings and member engagement.

Nice-to-haves
• Certified Case Manager (CCM).

• Experience with Texas STAR and CHIP case management.

• Experience working with members with serious mental illness (SMI).

• Experience in managed care.

• Bilingual in English/Spanish or Chinese/Mandarin.
