Job Overview:
We are seeking a dedicated professional experienced in Chronic Care Management (CCM) to join our team. The Care Manager (MA or LPN/LVN) is responsible for providing comprehensive, coordinated care to patients with multiple chronic conditions through technology and clinical service model. This role requires excellent communication, clinical skills, and a strong commitment to improving patient health outcomes. The Care Manager plays a crucial role in maintaining patient health and filling the gaps between their doctorÂs appointments. Skills and Qualifications:  LPN/LVN/CNA/MA: New Jersey Licensure in good standing - Required  Minimum 1 year of experience with CCM - Required  Fluency in English and Spanish - Required  Minimum 2 years of experience in the healthcare industry; preferably in a primary care providerÂs office - Required  Billing/RCM or health coach experience may be considered.  Knowledge of Medicare's CCM program guidelines and requirements  Experience with and comfortable in assessing the status of patients remotely.  Working knowledge of disease processes for common chronic illnesses (eg: Hypertension, Diabetes, COPD, CHF) with demonstrable ability to communicate issues related to it over the phone.  Clinically sound and accurate documentation skills.  Experience with and comfortable learning health portals and software applications.  Superior task management skills: organized, self-directed, and detail oriented.  Enterprising to find solutions when faced with problems.  Ability to work collaboratively with a team and manage multiple priorities and reliably meet deadlines.  Quiet setting without interruptions to work from. Reliable internet service and electricity.
Duties & Responsibilities
 Execute care plans for patients with chronic conditions in collaboration with the healthcare team.
 Proactive patient management via text, call, and electronic medical records.
 Triage patient conditions and respond accordingly in a timely manner
 Work closely with in-office providers and staff for guidance with management of day-to-day calls related to condition, medication, referral, and community resource management.
 Engage patients about their health by providing guidance, education, and encouragement.
 Coordinate care with other healthcare professionals, ensuring timely communication and collaboration.
 Actively review patient charts to identify care gaps and determine resolutions based on provided workflow
 Escalate patient care promptly according to pre-approved algorithm
 Advocate for patients' needs and preferences.
 Accurately and timely document patient interactions, interventions, and progress in the electronic health record system.
