Position Overview: The Care Manager with Behavioral Health Integration (BHI) experience provides comprehensive care coordination to patients, integrating behavioral health services within the primary care setting. This role involves engaging patients to support their physical and mental health, fostering teamwork with primary care providers and behavioral health consultants, and driving improved clinical outcomes. Through motivational interviewing, care planning, and patient education, the Care Manager ensures the delivery of holistic, high-quality care that aligns with the organizationÂs core values.

Key Responsibilities
 Patient Care Coordination
 Conduct behavioral health assessments using tools like PHQ-9 and GAD-7 under supervision.
 Collaborate with primary care providers and behavioral health consultants to develop individualized care plans.
 Conduct regular follow-ups to track patient progress, address barriers, and adjust care plans.
Patient Engagement and Education
 Use motivational interviewing techniques to activate patients and set achievable health goals.
 Educate patients on available resources and empower them to adhere to care plans.
Collaboration and Communication
 Act as a key contact for patients, ensuring seamless navigation of appointments and resources.
 Provide timely updates to providers and behavioral health consultants.
 Accurately document all patient interactions and care plans in the electronic health record (EHR).
Data Tracking and Reporting
 Monitor and report patient outcomes, including symptom improvements and adherence to care plans.
 Support quality assurance by maintaining accurate and timely documentation.
Program and Workflow Support
 Assist with implementing and refining BHI workflows to optimize care delivery.
 Identify gaps in services and recommend solutions for improved patient outcomes.
Key Performance Indicators (KPIs)
Patient Outcomes:
 Improvement in PHQ-9/GAD-7 scores.
 Percentage of patients with completed and documented care plans.
Engagement and Retention:
 Connect with 18 patients a day and / or make 60 outreach calls to patients per day
 Patient retention rates in the care management program.
 Number of successful patient follow-ups and interactions.
Operational Metrics:
 Timeliness and accuracy of documentation.
 Total patient interactions completed within set timeframes.
Satisfaction Metrics:
 Positive patient satisfaction scores.
 Feedback from team members on collaboration and communication.
Qualifications
 Licensed Vocational Nurse (LVN) or Medical Assistant (MA) certification required.
 Experience in behavioral health and/or patient care coordination preferred.
 Strong organizational, communication, and interpersonal skills.
 Proficiency in electronic health records (EHR) and data tracking systems.
 Bilingual (preferred, depending on patient population).
