Key Responsibilities

• Coordinate timely referrals for vital services, including housing assistance, vocational rehabilitation, specialized mental health care, medical referrals, and substance use treatment.
• Employ motivational interviewing techniques to educate and engage patients while managing follow-up care.
• Maintain and regularly update the patient registry.
• Accurately document care plans and clinical notes.
• Actively contribute to interdisciplinary care team meetings to ensure continuity of care and address patient needs.

Qualifications
• Active and unencumbered Licensed Vocational Nurse (LVN) license.

Compensation And Benefits
• Salary: (65,000 - )85,000 annually.
