Key Responsibilities
• Conduct comprehensive in-home evaluations to assess patient needs and develop tailored care plans.
• Collaborate with Physical Therapy Assistants (PTAs) to execute care plans effectively.
• Facilitate improvements in patient mobility and independence through targeted rehabilitation strategies.
• Maintain open communication with patients' physicians to ensure continuity of care and optimal outcomes.
• Evaluate and document patient progress, providing updates to medical professionals as necessary.
• Supervise a team of 2 PTAs, ensuring adherence to performance standards and best practices.

Required Skills
• Current, unrestricted California Physical Therapist License.
• Minimum of 1 year of experience supervising PTAs in a home health setting (preferred).
• Valid BLS/CPR Certification and California Driver’s License.
• Knowledge of local community resources and their relevance to patient care.
• Strong understanding of individual development, human behavior, and cultural influences in healthcare.
• Familiarity with state and federal homecare regulations (preferred).
• Experience working with high-acuity patients and interdisciplinary teams (highly desired).
• Proficiency in case management fundamentals (highly desired).
• Exceptional organizational, interpersonal, and leadership skills.

Qualifications
• Self-direction, empathy, and solid critical thinking capabilities are essential for this role.
• A post-secondary degree in Physical Therapy or an equivalent qualification is required.
