TheCommunity Health Workersupports theoperational and administrative functions of a multi-year programdesigned to shrink health disparities in the San Gabriel Valley.
This is a grant funded position with a one-year contract, with the possibility of extending until August 2026.
Responsibilities:
Conduct outreach in the San Gabriel ValleySupport planning and coordination of aprogram activities and operationsNavigate community resources and share them with community members in needLead community listening sessions and share public health information on topics such as COVID-19Coordinate and maintain applicable program schedules and agendas, which may include tracking deadlines, deliverables, communication with management and/or stakeholdersHelp deliver general welfare interventions to the communities across Los Angeles County, with emphasis on the San Gabriel ValleyUse software apps and tools to collect and reportrelevant data tracking community healthmetrics.Other duties as assigned by the Program and Area Managers Scope and Travel:
Extensive travel by foot and by car within the San Gabriel Valley (Cities of Rosemead, El Monte, South El Monte, Altadena, Pasadena), as well as travel to and from COREâs Los Angeles Headquarters (Chinatown) Qualifications:
Preference for candidate with bachelorâs degree in public health or health sciences, or at least two yearâs equivalent experience in relevant field Experience:
Minimum 1 year experience in public health, healthcare settings, community outreach or related fieldsExperience in community outreach, conducting surveys, collecting data, and reporting dataExperience working with community-based organizations, local government agencies, volunteer groups in LA County (preferred)Knowledge of community resources in LA County Skills and Competencies:
Ability to work independently and as part of a teamExcellent written and verbal communication skillsProficient in MS Office applicationsFlexible and adaptive to a frequently changing environmentExcellent organizational and problem-solving skillsClose attention to detail and a high level of accuracyBilingual in English and Spanish requiredAbility to walk 1-3 miles/day (reliable transportation required)Knowledgeable of HIPAA In accordance with CA law, the expected pay range for this CA position is posted with this position. The actual compensation will be determined based on experience and other factors permitted by law.
