Position Summary



The Pharmacist for Asante Physician Partners (APP) collaborates in patient care as a part of an interdisciplinary care team-based approach with physicians, other providers, nurses, and health care professionals with a goal of improving health care quality. In this role, the Pharmacist performs individual patient assessment of needs, identifies drug therapy problems, develops patient specific plan to optimize medication use, educates patient in use of medications, and conducts follow-up evaluations to determine patient outcome. Also performs routine tests, completes paperwork as appropriate, and fields and conducts patient calls as requested.



APP is a group of top medical professionals working together to provide comprehensive care to patients in Southern Oregon. We collaborate with other health care professionals throughout the area to offer a full array of quality care close to home, supported by Asante's full range of services and hospitals. Our team-based clinics operate on a Patient-Centered Primary Care Home model. This allows physicians to do what they do best: take care of patients while being supported by a team of specialty professionals.



Qualifications



Education



  • Bachelor's degree in pharmacy - or the equivalent in education and experience may be considered.

  • Doctorate degree in Pharmacy, preferred.

Experience



  • Experience in a similar pharmacy practice setting is desirable. Preferred Residency Experience PGY1 (preferred); PGY2 Ambulatory Care (optimally preferred).

Licensure/Certifications



  • RPH: Registered Pharmacist by Oregon Board of Pharmacy required upon start.

  • BLS: Basic Life Support for the Healthcare Provider certified through the American Heart Association (AHA) required upon start.

  • CDE: Certified Diabetes Educator, preferred.
