Northwest Kidney Centers seeks a Chronic Kidney Disease (CKD) Educator to join the Chronic Kidney Disease Program in a per-diem, hybrid position.

Purpose:

The Chronic Kidney Disease (CKD) Educator is responsible for providing education, support, and assistance to facilitate informed decision-making, access to care and successful self-management for pre-dialysis patients; with the goal of improving outcomes related to modality choice (including palliative care), access placement, and transplant. The position is responsible for teaching CKD classes, assisting in development and implementation of educational programs for patients, NKC staff, and the community. Internally, the CKD Educator serves as a liaison, consultant, and resource for the internal Interdisciplinary Team Members regarding dialysis patient education. Externally, this position will perform outreach/education to area Nephrologists, healthcare providers, individuals impacted by CKD, and the lay public.

Shift: Per-Diem/Variable, Mon-Fri.

Duties and Responsibilities:
  • Conduct modality education classes and individual sessions for CKD pre-dialysis patients in the areas of modality choice, preparations for dialysis, transplant, palliative care, and self-management of CKD.
  • Develop and maintain relationships with providers outside NKC, including nephrology offices, transplant centers, hospitals, and other agencies to promote and facilitate access to CKD education and services.
  • Give presentations and/or provide public speaking for community groups, agencies, NKC staff or other healthcare providers on CKD topics as assigned.
  • Review data and share findings related to CKD services provided.
  • Participate in curriculum development, implementation, and evaluation of materials and programs for patient education.
  • Assist in education and coordination of follow up for center patients who are interested in pursuing home modalities and/or transplant.
  • Performs relief, project, and committee assignments on an as needed basis. This includes coordinating and performing special project duties and responsibilities including projects related to improving patient outcomes. Chronic Kidney Disease (CKD)
  • Assist the Home Program Manager in establishing procedures and policies to implement and maintain CKD educational services.
  • Keep informed of current professional standards and practices especially those involving CKD state of the art practices and programs.
  • Perform CKD operational support duties alongside the CKD Program Assistant as directed by the Home Program Manager such as referral processing, patient scheduling, and preparing class materials.
  • Responsible for continuous review to improve personal work process with everyday continuous quality improvement.
  • Other duties as assigned.
The ideal candidate will:

The CKD Educator must a registered nurse or licensed social worker. 2 years of previous clinical patient education experience in the renal field is required. The CKD Educator must demonstrate competency in clinical practice, adult education, interpersonal communication skills, outreach/networking with stakeholders and leadership.

Physical Conditions:
  • Must be able to communicate effectively in English on the phone, in writing, via Telehealth and
  • Must be able to use a computer keyboard, mouse, and pull-down windows in an efficient manner. Must be able to demonstrate use of appropriate software after training. Proficiency with MS Word, PowerPoint, Planner, Access, WebEx and Excel is required.
  • Duties and responsibilities are performed in office, clinical dialysis unit, physician offices, other healthcare organizations, online, and other community settings.
  • Physical activities require the ability to write, talk, hear, keyboard.
  • The position requires visual acuity to read and write.
  • Must have a valid Washington State Driver's License, proof of insurance and ability to travel to all NKC facilities as needed.
  • Must have valid WA State Licensure in their field as applicable.
  • Must be able to teach and make public presentations.
