The Care Coordinator role is a critical member of the Landmark Interdisciplinary Team. This role reports to a Supervisor or the Director, Health Services. Responsibilities are to ensure a positive patient experience with Landmark, as well as to build good relationships with local provider groups to ensure that Landmark is a reliable and trusted partner in patient care. Employees are required to have flexibility to work an 8-hour shift schedule during our normal business hours of 08:30am  05:00pm EST. It may be necessary, given the business need, to work occasional overtime or weekends. Our offices are located at 77 Monroe Center Street NW, Suite 600, Grand Rapids, MI 49503 and 13919 S West Bay Shore Dr. STE G03 Traverse City, MI 49684. Employees will be required have flexibility to visit the office at least once per month for team meetings and/or training.  All Telecommuters will be required to adhere to United Health GroupÂs Telecommuter Policy. Primary Responsibilities:  Scheduling/rescheduling visits  Calendar management  Medical records  Consents/ROIs (release of information)  Informational packets  Regular contact with provider groups to collect clinical data.  Support patients in scheduling home visits.  Ensure that Landmark has all documentation available regarding patient conditions.  Support Landmark providers and Nurse Care Managers with operational and clinical tasks.  Handle escalated patient issues, ensuring the appropriate resources are involved.  Follow triage protocol when needed. Required Qualifications:  High School Diploma / GED  Must be 18 years or older  6+ months of experience as a Medical Assistant, Certified Nursing Assistant, Pharmacy Tech, or medical office assistant, or administrative role in a health care setting  Experience with handling customer service issue resolution  Basic proficiency with Microsoft Excel (open, enter data, edit, save), Word (opening, reading, saving files), intermediate proficiency with Outlook (email, calendar, scheduling)  Ability to work between the hours of 8:30am - 5:00pm EST, Monday  Friday. It may be necessary, given the business need, to work occasional overtime or weekends. Preferred Qualifications:  Prior experience working with triage protocol  Experience with escalated customer service issue resolution.
