In this Community Health Worker role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.
If you are in or near Wichita, KS and surrounding areas you will have the flexibility to telecommute as you take on some tough challenges. Expect to spend about 20% of your time in the field visiting our members in their homes or in long-term care facilities, within assigned territory.
Primary Responsibilities
 Complete contractual requirements for UTR members for Initial, Annual, 6mo, and Post Hospital Assessments ensuring UTR member receive timely contact and prevent liquidated damages due to late contacts for no response. Complete cold-knock activities for UTR member who have not responded to other attempts
 Complete 14- and 30-day follow-up contacts for members discharged from Hospitalization, ensuring memberÂs needs are met and supports are in place for member to remain in the community
 Obtain contractually required member signatures for Service Plan, Participant Interest Inventory, Authorized Rep, Designated Rep, and other required forms if the CC is not able to successfully obtain and/or has confirmed agreement from member/legal representative to utilize DocuSign
 Obtain contractually required Physician/RN statement and signature from PCP/RN allowing members to self-direct health maintenance activities
 Locate resources that impact the memberÂs medical needs such as SDOH resources (transportation, food, clothing, housing), identifying providers with capacity for new members, locating community resources and financial support for equipment, safety items (e.g. smoke detector), etc. Fulfilling SDOH impacts the memberÂs ability to remain in the community and prevents higher level of care placement
 Search for agency-direct caregiver to support the memberÂs needs and fulfill the memberÂs service hours
 Monitor members in the closure process and escalate closures to the State to ensure timely closing of identified members
 Obtain TBI therapy notes from agencies for all new and existing BI waiver members

Required Qualifications
 High School Diploma/GED (or higher)
 2+ years of experience working within the community health setting or in a health care role
 Resided within the local community for 2+ years
 Familiarity with the resources available in the community
 Field based experience assisting others with healthcare and/or social determinants of health needs
 Have a designated workspace inside the home with access to high-speed internet availability
 Access to reliable transportation that will enable you to travel in an assigned region to visit Medicaid members in their homes and / or other settings, including community centers, hospitals, or providers' offices
Preferred Qualifications
 Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant
 CHW Accreditation
 Field-based experience
 Case management experience
