Join our dynamic and innovative healthcare team as a Nurse Practitioner or Physician Assistant, dedicated to enhancing the lives of patients with chronic kidney disease. This pivotal role offers the opportunity to provide both in-home and virtual care, ensuring personalized support and management for those in need.
Key Responsibilities
 Conduct thorough assessments, including annual wellness exams, for patients in their homes and through virtual consultations.
 Educate and counsel patients and their families on available health resources and programs.
 Maintain meticulous patient records, documenting all pertinent medical histories, diagnoses, medications, and treatment plans.
 Collaborate effectively with primary care providers, specialists, and ancillary services to coordinate comprehensive patient care.
 Deliver evidence-based care aimed at minimizing hospitalizations while maximizing patient quality of life.
 Prescribe medications and order necessary tests in conjunction with the patientÂs physician.
 Address gaps in care by educating patients about preventive measures and facilitating access to diagnostic testing.
 Assist patients in utilizing educational resources for better health management.
 Participate in integrated care team meetings to ensure cohesive patient care.
 Undertake additional duties as required.
Required Skills
 Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
 Board certification for relevant licensure (NP: ANCC/AANP; PA: NCCPA).
 Current DEA certificate without restrictions.
 Basic Life Support (BLS) certification (assistance available if needed).
 Strong ability to function autonomously and independently.
 Valid driverÂs license, car insurance, and access to reliable transportation.
 Proficient in using medical instruments and equipment.
 Familiarity with computer-based data management programs and systems.
 Excellent verbal and written communication skills.
 Solid understanding of federal and state regulations, including HIPAA and OSHA.
 A minimum of 2 years' experience in direct patient care.
 Preferred: experience with Managed Care/IPA/Health Plan and conducting wellness visits (either virtual or in-home).
 Bilingual proficiency in English and Spanish is highly desirable.
Qualifications

 Board certification for appropriate licensure (NP: ANCC/AANP; PA: NCCPA).
 Minimum of 2 years direct patient care experience.
