What To Expect (Job Responsibilities)
• Engage with patients to navigate their care and address healthcare issues
• Collaborate with medical practice staff and the Case Management team
• Document patients' medical history and issues in the electronic health record (EHR)
• Implement and meet assigned Key Performance Indicators to improve patient outcomes
• Utilize various communication methods to engage high-risk patients

What Is Required (Qualifications)
• Certified or Registered Medical Assistant OR certified EMT
• Experience with medical terminology and understanding of primary care physician practices
• Proficiency in using electronic health records (EHRs) and care management analytics
• Exceptional written, visual, and verbal communication skills
• Ability to work independently as part of a patient's care team under supervision

How To Stand Out (Preferred Qualifications)
• Familiarity with electronic health records such as EPIC, Athena, or Cerner
• Experience in motivational interviewing and behavioral change management
• Strong critical listening skills and empathy in patient coordination
• Ability to manage multiple tasks while remaining detail-oriented
• Proven success in a remote work environment
