Key Responsibilities
 Deliver after-hours triage services to hospice patients, adhering to customized protocols that prioritize patient-centered care.
 Operate from a home-based setting, assisting families and patients during vital moments without the challenges of commuting.
 Maintain a part-time evening schedule (3:30 PM - 12:00 AM CST) with rotating shifts on Saturdays and Sundays.
 Access and document patient information utilizing electronic medical records (EMRs) and internal applications.
 Complete necessary shift preparation and post-call documentation accurately.
Required Skills
 Valid LPN License: Must possess or be willing to obtain a Compact LPN license (residency in a Compact US state required).
 Experience in End-of-Life Care: Previous exposure to hospice, palliative, or end-of-life care is crucial.
 Fast-Paced Environment: Experience in high-pressure settings such as Emergency Departments or critical care is advantageous.
 Technological Proficiency: Ability to navigate various technology applications and type efficiently in a dynamic environment.
 Communication Skills: Exceptional ability to engage empathetically with patients while maintaining adherence to protocols.
Qualifications
