PROGRAM SUMMARY:

At Auberle we are committed to helping families and individuals achieve stability and self-sufficiency through our housing programs. The Housing Case Manager will be responsible for providing supportive case management to participants experiencing homelessness or a housing crisis. Case Managers will meet participants where they are at, including travel throughout Allegheny County. The ideal candidate will have experience working with low-income families and individuals, as well as knowledge of affordable housing programs and resources. The Case Manager will be responsible for conducting intake interviews, assessing participant needs, developing service plans, and providing on-going case management and support services. The goal of this position is to help our clients obtain and maintain stable housing.

ESSENTIAL RESPONSIBILITIES: • Assist participants with transportation to view housing options, complete housing applications, and needs related to maintaining stability. • Assist participants to access community-based resources by making and coordinating referrals for all housing, aftercare, medical, and other needed supplementary health and human services. • Monitor service delivery and identify barriers to services through regular contact with participants, providers and treatment team. • Meet weekly and/or as needed with Program Supervisor for supervision to discuss program development and implementation of policies and procedures applicable to the provision of services, staff development and participant’s progress. • Participate in assigned team meetings and consultations as requested, as well as staff development and training activities. • Acts as landlord’s primary liaison to the program, access landlord networks, understand landlord perspectives, market the program, and negotiate leases on behalf of the participants, building lasting landlord relationships. • Promotes individual professional growth and development through certification, mentoring/precepting, and/or participation on department committees. • Supports department-based goals that contribute to the success of the organization. • Maintain productivity standard & other duties as assigned.

QUALIFICATIONS/REQUIREMENTS: • Experience working with displaced, underserved, and vulnerable populations. • Experience working with HUD, Homeless Advisory Board and HMIS strongly preferred. • Bachelor’s Degree preferred with at least one year of administrative experience and/or equivalent skills & experience working within a housing or shelter program. • Strong communication skills both written and verbal. • Must be able to work one-on-one with participants. • Must be able to pass background check including FBI clearance and ACT 33/34 clearances. • Must possess a valid Pennsylvania’ driver’s license, reliable vehicle, registration and insurance. • Required flexible hours, including evening and weekends.
